Pride of Formby Awards - Have your say

It is time to nominate your community hero for a Pride of Formby Award. There are several categories to choose from including teacher of the year, fundraiser of the year, good neighbour, special young person and others.

To nominate someone, you can go on-line a:

www.surveymonkey.co.uk/r/POFA and complete the form on-line, download the nomination form at www.formbyparishcouncil.org.uk or call into Formby Library to pick up a paper copy.

Completed forms should be returned to Formby Parish Council at Formby Library by Friday 7 September 2018.
